Nevada unveils a new public insurance program as federal health reforms face turbulence

Nevada’s new public option health plans have enrolled more than 10,000 residents since launching last fall—far below the 35,000 enrollees state officials had projected. Nevada is the third state to implement a public option, following Washington and Colorado. These programs aim to expand access to affordable coverage through regulated private-public partnerships rather than fully government-run insurance.

In Nevada, insurers offering the “Battle Born State Plans” must reduce premiums by 15% over four years compared with a benchmark silver plan on Nevada Health Link. However, experts say the savings may not offset rising costs caused by the expiration of enhanced subsidies under the Affordable Care Act. Nearly 95,000 Nevadans previously benefited from those subsidies, which significantly lowered monthly premiums. Nationwide, enrollment has declined following their expiration, and millions are projected to lose coverage.

Additional federal policy changes—such as stricter income verification, shorter enrollment windows, and the end of automatic reenrollment under legislation signed by Donald Trump—are expected to further reduce marketplace participation. Analysts estimate that up to 100,000 Nevadans could lose coverage as a result.

Other states have faced mixed results. Washington initially struggled with low enrollment but later expanded participation after requiring hospital contracts with public option plans. Colorado’s public option has grown steadily, though insurers have not fully met premium reduction targets. Meanwhile, states like Minnesota, Maine, and New Mexico have paused or abandoned similar proposals.

Despite challenges, Nevada officials believe enrollment will increase as awareness grows. Still, researchers caution that while public options may improve affordability at the state level, they cannot fully compensate for the impact of sweeping federal changes on health coverage.
